For example: paying privately and never assessed; Checklist or DST completed; CHC refused; funding withdrawn; appeal underway; or the person has died.
What period are you concerned about?
Approximate dates are enough initially. If the issue concerns an estate, tell us whether you are the executor or administrator.
What evidence already exists?
Decision letters, Checklist or DST documents, care plans, nursing records, hospital records and invoices can all help us understand the procedural history.
